Information we collect

Information you provide. When you submit our contact / insurance-verification form or otherwise reach out to us, we may collect your name, phone number, email address, date of birth, insurance carrier and member ID, the reason for your inquiry, and any details you include in a message.

Information collected automatically. Like most websites, we automatically collect limited technical data such as your IP address, browser and device type, pages viewed, and referring pages. We collect this through cookies and similar technologies, including Google Analytics.

How we use your information

Protected health information & HIPAA

PRS is a healthcare provider, and health information you share with us as a patient is protected under the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law. Our separate Notice of Privacy Practices, provided at intake, governs how we handle protected health information (PHI) in our clinical care. Please avoid including sensitive medical details in website form messages; we only need enough to contact you and begin the conversation securely.

You can opt out using the Google Analytics opt-out browser add-on, or by disabling cookies in your browser. Most browsers also let you send a "Do Not Track" signal.

How we share information

We do not sell your personal information. We share it only:

Data security

We use reasonable administrative, technical, and physical safeguards to protect the information we collect. No method of transmission or storage is completely secure, however, and we cannot guarantee absolute security.
